Success stories (and horror stories) with stakeholders

Has your agency/group been involved in a process that successfully involved stakeholders (citizens, advocacy groups, non-transportation public agencies, transit providers, etc.) in meaningful decision making? What was the situation, and why did it work so well? What methods/techniques/tools were used to both gather and incorporate the stakeholders’ input?

Likewise, do you know of examples where stakeholders were not well-integrated into the process, and it caused problems? What kinds of problems arose as a result? What could have been done differently, and how might that have helped?
